VITAL RECORDS REQUEST INFORMATION

Birth Certificate - $10/certified copy
Who is eligible to obtain a Birth Certified copy?
  • person named on Birth certificate
  • parent of the person named on Birth Cert. (requesting parents name must be on birth certificate)
  • only by order of a NYS court may a spouse, child, or other person obtain a copy of birth certificate

Death Certificate - $10/certified copy
Who is eligible to obtain a Death Certificate copy?
  • The Spouse, parent, child, or sibling of the deceased
  • Other persons who have a documented lawful right or claim, documented medical need, or NYS court order

Genealogy Record - Search fee $11/search
Vital Records registration started in New York State outside of NYC in 1881.
Uncertified copies of the following types of records for genealogy purposes:
  • Birth Certificates- if on file for at least 75 years & the person whose name is on the birth cert. is known to be deceased.
  • Death Certificates- if on file for at least 50 years
  • Marriage Certificates- If on file for at least 50 years & both spouses are known to be deceased.
 *No ID or proof of relationship is needed for Genealogy Searches only*
 
​Marriage Certificate - Local Application/Search $10/certified copy
Who is eligible to obtain a Marriage Certified copy?
  • Spouses
  • Other persons who have a documented judicial or other proper purpose, or NYS Court order​ ​

** IDENTIFICATION REQUIREMENTS FOR COPIES OF BIRTH, DEATH, AND MARRIAGE CERTIFICATES**
Applications must be submitted with copies of One (1) of the following:
  • Drivers License
  • State issued non-driver photo ID card
  • Passport
  • U.S. Military issued photo ID
 
OR: Two (2) of the following:
  • Utility or phone Bills
  • Letter from a Government agency dated within the last 6 months

** MAILED REQUESTS**
For Vital records and Marriage Certificates not classified as “genealogy”, request must be notarized with copy of drivers license, date of record or date range, and names of person/s on record.

Must include address to mail document to and a phone number of the person requesting. A check or money order for the record must be included. Must include attached application for record as well.
